I hereby consent to emergency medical treatment by the FUN-damental Basketball Camp, and appoint the camp director or camp trainer to act in my behalf in authorizing emergency medical attention beyond that maintained by the camp.  I hereby waive and release the camp from any and all liability for injuries while at the camp or arising from travel to and from camp.  Camp will not be responsible for medical costs.

IMPORTANT! Make sure you print out and bring in both (2) pages for this form. (Including Immunization History).
3. Please send completed application, emergency consent forms and $125 non refundable deposit (if you do not pay with a credit card online) to: FUN-damental Basketball Camp Inc. 7904 E. Ridge Pt. Drive, Fayetteville, NY 13066. Full payment for camp may be made. New York Campers please also send the Camp Health Examination form with application or prior to camp.
4. A confirmation packet with important information will be sent prior to camp arrival.
5. Payment in full must be made by June 15. Any payments received after that date must be made by certified check , postal money order, or credit card from our web site.
